Today, we’re taking a closer look at TB-500, also known as Thymosin Beta-4 (TB4), a peptide frequently used to support injury recovery, tissue repair, and healing.
What Is TB-500?
TB-500 is a synthetic peptide derived from a naturally occurring protein called Thymosin Beta-4 (TB4). This protein is produced throughout the body and is upregulated in response to injury and tissue damage.
Cells involved in healing—including platelets, macrophages, and endothelial cells—contain higher concentrations of TB4, suggesting it plays an important role in the body’s natural repair processes.
In peptide therapy, TB-500 is often paired with BPC-157 due to their complementary effects on recovery and regeneration.
How Does TB-500 Work?
The healing process is incredibly complex. Following an injury, the body must coordinate inflammation, immune response, blood flow, tissue remodeling, and ultimately repair.
Research suggests TB4, the parent molecule of TB-500, may influence several key aspects of this process:
Supports Cell Migration
One of TB4’s most studied functions is its ability to bind to a protein called actin. This interaction may help regulate the movement of immune cells, repair cells, and vascular cells to areas of injury where they are needed most.
Promotes Healthy Blood Vessel Growth
TB-500 appears to support angiogenesis, the formation of new blood vessels. Improved blood flow may help deliver nutrients and oxygen to injured tissue while assisting with waste and debris removal.
May Help Regulate Inflammation
Laboratory studies suggest TB-500 may inhibit NF-kB, a major regulator of inflammation. This may help shift damaged tissue from a prolonged inflammatory state toward repair and recovery.
Supports Collagen Remodeling
Animal studies have demonstrated improvements in collagen organization and scar formation, both of which are critical components of effective tissue healing.
Because TB-500 is administered as an injectable peptide, its effects are considered systemic. Rather than targeting a specific shoulder, knee, or tendon, the goal is to support the body’s overall response to injury and tissue remodeling.
What Does the Research Say About TB-500?
TB-500 remains one of the more intriguing peptides because it does have some human research behind it.
Pharmaceutical formulations of full-length Thymosin Beta-4 have been studied for:
- Ocular surface disorders
- Corneal wound healing
- Recovery following myocardial infarction (heart attack)
- Reduction of cardiac fibrosis
However, there are currently no human clinical trials evaluating TB-500 for tendon injuries, ligament healing, muscle recovery, or post-surgical repair—the areas where it is most commonly discussed in peptide therapy circles.
Most of the evidence supporting these applications comes from laboratory research, animal studies, and our growing understanding of the biological mechanisms involved in tissue repair.
While the early data is promising, more human studies are needed.
TB-500 Dosage and Clinical Use
In our practice, TB-500 is most commonly used alongside BPC-157 to support healing following:
- Musculoskeletal injuries
- Tendon and ligament issues
- Surgical recovery
- Chronic inflammatory conditions
- Post-viral recovery
- Arthritis and other inflammatory disorders
TB-500 is typically dosed twice weekly when used alone, though some protocols utilize daily dosing when combined with BPC-157.
Treatment is generally cycled for four to six weeks, with dosage individualized based on the patient’s goals, injury type, and overall health status.
Although oral forms of TB-500 are available, we currently do not recommend them due to uncertainty regarding absorption and survival through the digestive tract.
It’s also important to note that both TB-500 and BPC-157 are currently prohibited by the World Anti-Doping Agency (WADA).
Is TB-500 Safe?
As with most peptides, long-term human safety data remains limited.
In our clinical experience, we have not observed significant adverse effects with short-term, appropriately supervised use. However, because long-term safety remains unclear, we generally recommend:
- Using TB-500 for defined treatment periods
- Cycling off after four to six weeks
- Avoiding continuous, indefinite use
- Reserving treatment for specific recovery and repair goals
We also strongly recommend obtaining TB-500 through an experienced clinician who sources products from reputable compounding pharmacies.
We do not recommend purchasing peptides from direct-to-consumer sources due to concerns surrounding purity, quality control, and dosing accuracy.
TB-500 Is Just One Part of the Recovery Equation
While TB-500 may support the body’s natural healing processes, no peptide can replace the fundamentals of recovery.
Successful healing after injury or surgery still depends on:
- Quality sleep
- Adequate protein and nutrition
- Appropriate rehabilitation
- Joint and tissue protection
- Maintaining mobility and range of motion
- Managing chronic inflammation
- Supporting overall health

TB-500 is an exciting peptide with significant potential in the areas of tissue repair, recovery, and regenerative medicine. While human research remains limited, its biological mechanisms and early evidence make it an intriguing option for supporting healing after injury or surgery.
We frequently use TB-500—especially when combined with BPC-157—as part of a comprehensive recovery strategy. As we age, the body’s repair systems become less efficient, and chronic inflammation can further impair healing. Supporting these pathways safely and strategically may help improve recovery outcomes and overall resilience.
Like any therapy, TB-500 works best when used intentionally, under medical supervision, and alongside the lifestyle habits that create the foundation for healing.
