Part of being human involves injury and repair: joint damage, muscle strains, surgery, arthritis, and chronic inflammation. These events are so common that our bodies have developed a very elegant and complex way of healing. Immediately after an injury, the body sends out numerous signals for inflammation, blood vessel formation, immune cell migration, and tissue remodeling. When we are young, this process is robust, but aging, chronic disease, poor circulation, and repetitive injuries can create an environment that impairs the body’s ability to heal.
Many patients find themselves in this scenario and struggle knowing what to do next. More physical therapy? More ibuprofen? More time?
This is where I will often turn to BPC-157 peptide therapy, especially BPC-157. When you’re already doing what you can to promote healing and recovery and want to optimize the body’s healing response, BPC-157 and TB-500 (stay tuned!) can serve as a robust complement to your healing journey. However, these peptides are largely misunderstood and poorly discussed in today’s hyper-wellness space.
As a physician who uses these peptides daily as part of a personalized regenerative medicine approach, I want to give you a thorough overview of:

Before we dive in, it’s important to understand where the science stands. BPC-157 is not FDA-approved for human use, and we currently lack large, high-quality human clinical trials proving its effectiveness. Much of our understanding comes from laboratory research, animal studies, and growing clinical experience. But it is important to remember the common adage, “Absence of evidence does not mean evidence of absence.”
Just because the peptide doesn’t have a lot of human studies doesn’t make it worthless. Throughout this article, I’ll distinguish between what we know, what we think, and what we commonly observe in practice.
What Is BPC-157 and How Does It Work?
BPC-157 is a synthetic peptide derived from a natural molecule aptly named Body Protective Compound (i.e., BPC) that seems to protect and repair the gastrointestinal lining in animal studies. Though we do not have studies on exactly how BPC-157 works in humans, laboratory and animal studies suggest that BPC-157 supports the body’s natural healing response through several mechanisms.
Increasing Blood Flow
BPC-157 appears to increase blood flow to injured tissues. This is mostly through the formation of new blood vessels as well as enhanced nitric oxide signaling, a key vascular molecule that helps dilate blood vessels. By increasing circulation, injured tissues receive more oxygen, nutrients, and repair cells.
Supporting Connective Tissue Repair
BPC-157 appears to stimulate fibroblasts and enhance collagen formation, which may support tendon, ligament, and connective tissue healing.
Helping Regulate Inflammation
Inflammation is beneficial after an acute injury, but chronic inflammation can inhibit true repair and weaken joints, tendons, ligaments, and muscles over time. BPC-157 appears to help balance pro-inflammatory signaling pathways, helping the body move beyond a prolonged inflammatory state.
Since BPC-157 is derived from a naturally occurring gastric protein, it seems to have particular potency with gut health and gastrointestinal injuries, with laboratory and animal data supporting effects in gastric ulcer healing, gut barrier integrity, and inflammatory bowel diseases.
Combined, BPC-157 helps improve the environment surrounding injured tissues, supporting the body’s natural healing process.
How to Take BPC-157
We strongly discourage the purchase of peptides online, also referred to as the grey-market peptide market.
Firstly, grey-market peptides lack the necessary safeguards when it comes to quality and safety. When injecting something into your body, you want to make sure it is both pure and sterile.
Secondly, direct-to-consumer peptides lack guidance around dosing and monitoring—very important aspects when dealing with the human body.

BPC-157 comes in two forms:
- Subcutaneous injection
- Oral capsule
Interestingly, BPC-157 is one of the only peptides available in an effective oral capsule form, likely because of its natural affinity for the gastrointestinal tract.
Oral BPC-157
We use the oral formulation primarily when trying to influence gut-specific healing, including:
- Stomach ulcers
- Gut wall permeability
- Crohn’s disease
- Ulcerative colitis
- Celiac disease
- Post-infectious diarrhea
- Microscopic colitis
We may also consider oral BPC-157 when conditions appear to be influenced by gut inflammation, including psoriasis, rosacea, mood disorders, and cognitive changes.
Injectable BPC-157
Using BPC-157 as a subcutaneous injection is our preferred route for tendon, ligament, muscle, and post-surgical healing.
These injections use a very small needle to place BPC-157 into the fat just beneath the skin, allowing circulation to distribute the peptide throughout the body.
Many recommend injecting BPC-157 directly near the site of injury, but we don’t see much utility in this practice. BPC-157 functions as a signaling peptide, activating healing pathways where injury already exists.
How to Maximize the Benefits of BPC-157
It’s important to remember that BPC-157—and all peptides, for that matter—are simply tools in a toolbox. They are not magic and certainly are not powerful enough to completely alter your biology by themselves.
The only way to truly enhance the effectiveness of BPC-157 peptide therapy is to use it alongside a strong health foundation.
For example:
- Remove inflammatory foods and obvious food triggers.
- Reduce alcohol consumption.
- Support healing with nutrients like zinc, glutamine, butyrate, NAC, glycine, and vitamin C.
- Continue physical therapy.
- Prioritize rest and recovery.
BPC-157 works best when the body is already in a healing state.
BPC-157 and Safety
To be honest, we do not have extensive information regarding the long-term safety profile of BPC-157.
Because BPC-157 appears to influence angiogenesis (blood vessel growth) in preclinical studies, there are theoretical concerns regarding its use in certain situations, including cancer. These concerns are based on biological plausibility rather than demonstrated human harm.
With that in mind, I recommend avoiding BPC-157 if you:
- Have active cancer or have recently undergone cancer treatment.
- Have certain eye diseases such as wet macular degeneration, diabetic retinopathy, or macular edema.
- Are pregnant or breastfeeding.
Our clinic practices peptide cycling, typically using peptides for four to twelve weeks before taking a break. We do this to reduce the possibility of peptide tolerance while providing a buffer against potential risks from chronic exposure.
How Does BPC-157 Fit Into a Healing Plan?
BPC-157 is a foundational peptide because its mechanisms appear to support tissue repair in a unique way while complementing more traditional approaches to healing.
Its flexibility with both oral and injectable dosing makes it an excellent introduction to physician-guided peptide therapy for appropriate patients.
BPC-157 is not magic, but we do believe it can play a meaningful role in supporting recovery from certain injuries and gastrointestinal conditions when used responsibly.
