When most people receive their cholesterol test results, they immediately look at one number: total cholesterol.
The problem? Total cholesterol tells only a small part of the story.

- Sometimes VLDL cholesterol

What’s important to understand is that heart disease isn’t caused by one high cholesterol reading. It’s the result of years—even decades—of cholesterol-containing particles circulating through your arteries and contributing to plaque buildup.

1. Stop Focusing on Total Cholesterol

“My total cholesterol is 220. Should I be worried?”
The truth is, total cholesterol is one of the least useful numbers on your report.
Years ago, it was one of the few measurements available. Today, we have much more informative markers that better predict cardiovascular risk.
Total cholesterol combines several different cholesterol types into one value, making it difficult to know what’s actually happening inside your body.
Instead of focusing on total cholesterol, pay closer attention to:
- LDL cholesterol
- Non-HDL cholesterol
- Triglycerides
- ApoB (when available)
These markers provide a much clearer picture of your heart health.
2. A High HDL Isn’t a Free Pass
HDL has long been called the “good cholesterol,” leading many people to believe a high HDL protects them from heart disease.
Unfortunately, it’s not that simple.
A low HDL can indicate metabolic dysfunction and insulin resistance, making it an important warning sign.
However, once HDL reaches a healthy level (generally above 50 mg/dL), having an even higher number doesn’t necessarily provide additional protection.
In fact, research suggests that very high HDL levels may actually be associated with increased cardiovascular risk in some individuals, likely because the HDL particles aren’t functioning normally.
A healthy HDL does not cancel out elevated LDL cholesterol or ApoB.
3. Triglycerides Tell You About Metabolic Health
If there’s one number on the standard cholesterol panel that deserves more attention, it’s triglycerides.

When you consistently consume more calories than your body needs, excess energy is converted into triglycerides and stored in fat tissue.

A triglyceride level above 150 mg/dL is considered elevated.
However, many preventive medicine physicians prefer to see triglycerides below 100 mg/dL, since cardiovascular risk begins increasing well before they reach the upper limit of “normal.”
Very high triglycerides (greater than 500 mg/dL) require prompt medical attention because they significantly increase the risk of pancreatitis.
4. LDL Cholesterol Is Helpful—But It’s No Longer the Best Marker
LDL cholesterol remains an important part of cardiovascular risk assessment.
However, LDL-C measures how much cholesterol is inside LDL particles—not how many particles are circulating.
Current research shows that the number of cholesterol-carrying particles may be more important than the amount of cholesterol inside them.
General LDL goals include:
- Below 100 mg/dL for most healthy adults
- Below 70 mg/dL for individuals with increased cardiovascular risk
- Below 55 mg/dL for people with known plaque or previous cardiovascular events
It’s also important to remember that LDL becomes less reliable when triglycerides are elevated.
5. Non-HDL Cholesterol May Be the Best Number on a Standard Lipid Panel
If your doctor only orders a standard cholesterol panel, non-HDL cholesterol is often the most useful measurement.
Non-HDL cholesterol includes all cholesterol particles that contribute to plaque formation—not just LDL.
General targets include:
- Under 130 mg/dL for most adults
- Under 100 mg/dL for higher-risk patients
- Under 85 mg/dL for patients with known cardiovascular disease
Because it captures more cholesterol-containing particles, non-HDL cholesterol is often a stronger predictor of cardiovascular risk than LDL alone.
6. ApoB: The Most Important Cholesterol Test You’ve Probably Never Heard Of
If there’s one cholesterol marker gaining attention among preventive cardiologists, it’s Apolipoprotein B (ApoB).
ApoB measures the actual number of cholesterol-containing particles capable of entering artery walls.
Think about it this way:
Imagine two swimming pools.
One is filled with beach balls.
The other is filled with golf balls.
Both pools occupy the same volume—but one contains far more individual objects.
That’s exactly how cholesterol particles work.
LDL cholesterol measures the amount of cholesterol inside the particles.
ApoB measures the number of particles themselves.
More particles mean more opportunities for plaque to develop.
General ApoB targets include:
- Below 90 mg/dL for healthy adults
- Below 70-80 mg/dL if cardiovascular risk factors are present
- Below 60 mg/dL for patients with established cardiovascular disease
ApoB is increasingly recognized as one of the best predictors of future heart disease.
7. Heart Disease Develops Over Time
Heart disease rarely develops overnight.
It is the result of decades of exposure to elevated cholesterol particles, high blood pressure, insulin resistance, diabetes, smoking, chronic inflammation, and other risk factors.
That’s why waiting until cholesterol becomes “high enough” to treat can allow plaque to continue developing for years.
The earlier cholesterol-containing particles are controlled, the lower the lifetime risk of cardiovascular disease.
For younger adults especially, prevention today can dramatically reduce future heart attacks and strokes.
How to Track Your Cholesterol Over Time
Instead of watching total cholesterol, focus on trends that truly matter.
The most useful markers to follow include:
- ApoB
- Non-HDL cholesterol
- LDL cholesterol
- Triglycerides
If triglycerides are elevated, improving metabolic health through nutrition, physical activity, weight management, and insulin sensitivity often improves several cholesterol markers simultaneously.
It’s also worth asking your healthcare provider about Lp(a), an inherited cholesterol-related risk factor that only needs to be measured once in most people.
